Ansonia and Derby Parents: We Need Your Help!

The Yale-Griffin Prevention Research Center (Y-G PRC), in collaboration with the Naugatuck Valley Health District, is conducting a survey to understand communication channels and residents’ thoughts about routine childhood vaccines. This information will help to improve vaccine confidence and uptake efforts. You are being invited to participate in this survey because you are a parent or guardian who lives in Ansonia or Derby.

The survey will ask about the ways you receive information and your thoughts and experiences with routine childhood vaccines. The survey will take approximately 5-10 minutes. It is available in English and Spanish. You can take the survey online using Qualtrics. The survey is anonymous. All information you provide will be kept confidential within the Yale-Griffin PRC team. Participation in the survey is completely voluntary. You may choose to stop at any point or skip any questions. Your participation will not affect your eligibility for any current or future programs or services offered by Y-G PRC or the Naugatuck Valley Health District.

Your personal information will be kept completely confidential and stored securely. All survey responses will be reported together so that it is not possible to know any individual’s answers. Any future reports or publications will not identify you or include your personal information. After this study is over, all identifying information will be destroyed.
